Telling us something is wrong
If something about your supports isn’t right, we would rather know. Most things are sorted out quickly by talking to your support worker or coordinator, and you are welcome to start there. If you would prefer not to, or you have already tried, you can use the form below.
Making a complaint will not affect the supports you receive, and you will not be treated differently for raising one. You are welcome to have a family member, friend, support coordinator or an independent advocate help you, or make the complaint on your behalf.
What happens next
- We let you know we have received it, within 2 business days.
- We look into it, and keep you updated while we do.
- We aim to resolve straightforward matters within 10 business days, and more complex ones within 30. If we need longer, we will write to you and explain why.
- We tell you the outcome and what we have changed. If you aren’t satisfied you can ask us to review it, or go to the NDIS Commission at any time.

Only the first question is required. Fill in as much or as little as you like.
Thank you for telling us
